| About the Midwest Fall CupLakota FC would like to invite you to the MIDWEST FALL CUP. This is our fifth consecutive year. The tournament will be held at the premier venue in the Tri-State area: Voice of America Metropark in West Chester Township, Ohio. The Midwest Fall Cup is designed to offer B, C, and D level competitive youth soccer teams ages U7-U15 the opportunity to compete with similar level teams at a top-notch facility. Each team is guaranteed 3 matches. The mid-September climate in our area offers some fantastic days for families to enjoy the youth soccer experience. The Voice of America Athletic Complex features 20 natural grass fields and two lighted turf fields. The park setting, nearby trails, playground, and lake activities also provides added tournament benefits that your family can enjoy before, during, and after the times that your games take place! The Midwest Fall Cup tournament committee reserves the right of acceptance, denial or placement of any team. The tournament reserves the rights to consolidate age groups and divisions are as follows Black (Upper), Red (Middle), Gray (Lower) . Teams are selected based on competitiveness of the team within the bracket and timeliness of the application, so apply early! Registration paperwork submitted via mail or email. Mail check to Lakota Futbol Club, PO Box 212, West Chester, Ohio 45069. There will be game day check-in at the Headquarters tent 45 minutes before your first game of the tournament. The 2023 Midwest Fall Cup Tournament Committee Jen Marks, Tournament Director tournament@lakotafc.org SPECIAL REQUESTS: The tournament committee works very hard at scheduling and placing teams according to age, division and according to tournament application, it is your responsibility to fill out application completely and correctly depending on your past league and tournament results. Requests: We do try to accommodate coaching requests for coaches coaching more than one team but sometimes the schedule request is not possible. Any special requests should be made in the Notes section of the application. IMPORTANT ISSUE: CONCUSSION LAW In order to protect young athletes, the State of Ohio has passed a concussion law, commonly called the “Return to Play” law. Therefore, all Ohio Tournaments are responsible to monitor and track compliance with the concussion training requirements of the Return to Play law. See O.R.C. 3707.511(C)(1). Tournaments are required to collect and maintain certificates of compliance from the coaches. 1. All coaches MUST complete the Concussion Training Course from either NFHS (preferred) or CDC and submit a COPY of the completed certificate to the tournament. Both online courses are FREE of charge! See the links for the Concussion Form after you CONFIRM acceptance on the forms list. 2. Coaches must also carry their certificate COPY of compliance with them during the event. 3. Coaches are also required per Ohio law, to have distributed a COPY of the Parent Concussion Information Sheet to the parents of their team players. If these 3 items are not satisfied, those coaches will NOT be able to coach at this event. If a player is believed, by either a coach, referee or tournament official, to exhibit the signs, symptoms or behaviors of concussion during the tournament, that player shall be prohibited from continuing to play that same calendar day and player card will be pulled and may not play until they have received clearance to “return to play” by a physician or other authorized medical personnel. IMPORTANT ISSUE: Lindsay’s Law Ohio Lindsay’s Law, Ohio Revised Code 3313.5310, 3707.58 and 3707.59 went into effect on August 1, 2017.
